Each CareLink client has a highly trained, caring Nurse Care Coordinator to assess, establish and oversee an in-home care system to meet individual needs. If skilled nursing care is required, our nurse can provide personalized services such as assessments and medications management. If supportive services are part of a client’s plan, the Nurse Care Coordinator works with the individual and family to make sure personalized needs are met. Each client’s plan is customized based on individual situations.
